This quick guide supports GPs in preparing Mental Health Treatment Plans for patients referred to iflow Psychology in Gladesville. It outlines eligibility, required documentation, session entitlements, and how to forward referrals securely to our clinic.

1. Who Is Eligible for a Mental Health Treatment Plan?

1. Eligibility

A Mental Health Treatment Plan can be completed for patients presenting with symptoms of:

  • Anxiety disorders

  • Depression

  • Trauma-related concerns

  • Stress or adjustment difficulties

  • Sleep issues

  • Grief and loss

  • Other diagnosable mental health conditions under the DSM-5

 

Patients may be eligible for Medicare rebates for psychological therapy.

2. What to Include in a GP Mental Health Treatment Plan

Patient Details

  • Full name

  • Date of birth

  • Contact information

 

Clinical Information

  • Provisional diagnosis

  • Presenting concerns

  • Relevant history (psychiatric, medical, psychosocial)

  • Risk assessment (if applicable)

 

Treatment Recommendations

  • Requested number of sessions (typically 6 sessions initially)

  • Goals for psychological treatment

  • Any additional considerations

3. MHTP Session Entitlements

  • Initial referral: Up to 6 Medicare-rebated sessions

  • Review: After the first course of therapy, GPs may review the plan

  • Additional sessions: Up to 4 more sessions (total of 10 per calendar year)

4. How to Send an MHTP to iflow Psychology

GPs can forward the referral via:

A copy may also be given to the patient to bring to their first session.

5. After You Send the Referral

  • Our admin team acknowledges referrals within one business day

  • We contact the patient directly to arrange an appointment

  • With patient consent, GPs receive periodic progress updates as required by Medicare

6. When a Review MHTP is Needed

A review MHTP may be appropriate when:

  • The initial 6 sessions are completed

  • Additional sessions are clinically indicated

  • Treatment goals require updating

  • Circumstances or symptoms have changed

7. Telehealth Eligibility for MHTP Patients

Medicare rebates for telehealth are available if the patient meets criteria.
